CADDRA Membership Information



Membership Types
Please note that as of July 1, 2010 all CADDRA memberships will be subject to the new Harmonized Sales Tax (HST) rate of 15%


Full Membership

$200.00 + HST annually


Open to physicians or specialists with Canadian certification to practice medicine in Canada. Full Members are entitled to voting privileges.

Please supply a copy of your current medical licence or temporary provisional licence.


International Membership:
$150.00 + HST annually

Open to physicians who are non-residents of Canada, who have been certified by other than a Canadian certifying body. International Members receive full benefits but are not entitled to voting privileges.

Please supply a copy of your licence to practice medicine and a letter from a national medical society with your application.


Associate Membership:
$150.00 + HST annually

This is open to certified health care professionals such as psychologists, social workers, physician assistants, occupational therapists and nurses who have an interest in the field of ADHD. Associate members receive full benefits but are not entitled to voting privileges.

Please supply a copy of your certification or licence to practice.


Resident:
$50.00 + HST annually


Open to graduate physicians engaged in postgraduate training.  Residents receive full benefits but are not entitled to voting privileges.

Please have your application signed by your department head or program director

  • Triannual newsletters
  • Voting privileges (full members only)
  • 10% discount off the cost to attend the annual CADDRA Conference



Processing Applications

We look forward to welcoming you as a member. When we receive your completed application and appropriate fees, you will be considered a provisional member of CADDRA and you will be able to use your 10% discount towards your conference registration.



Application Process

  1. Each application will be checked for references, signature and completeness. Your office will be contacted to verify your information.
  2. If the application is complete, the applicant will be notified by email of his/her acceptance as a provisional member of CADDRA. A temporary provisional password will be issued that may be used to register for the conference and receive a 10% discount.
  3. The application will then be forwarded to the CADDRA membership committee for processing. When the membership committee has completed its review, it is returned to the membership department, who will then notify the applicant of final acceptance as a member and a permanent membership will be issued.

You will be notified in writing when your application has been processed and approved. You will then receive a membership package, along with a permanent member's number, in the mail.

1.    Download and print the appropriate application form:
2. Complete the form in its entirety, and ensure that it is signed.
3. Fill out the payment options form and include your signature if paying by Visa or MasterCard.
4. Attach all required documentation such as licences and certifications to the application form.
5. Complete reference information, including address, telephone number and email.

CADDRA requires that its members continue to conform to standards of high ethical and professional standing. If CADDRA learns that any information stated in your application is false or if,
after the date of application, circumstances that affect ethical and professional standards change, there may be grounds for suspension or revocation of membership.
